Conditional Discharge Survey

The Disability Rights Center – NH’s PAIMI Advisory Council is interested in learning more about conditional discharges in NH. In a conditional discharge, the patient must voluntarily agree to conditions in order to be discharged from New Hampshire Hospital. These conditions often include:

  • Complying with treatment from the community mental health center (CMHC);
  • Taking medications; and
  • Going to appointments.

If the individual doesn’t follow the above conditions, the conditional discharge may be revoked, and the individual may be sent back to the hospital against their wishes. It is our hope that the findings from this survey can be used to reform the conditional discharge process to better serve people with mental illness.
