Licensing Lookup

For All Types of Facilities

Step 1: Go to New Hampshire Online Licensing: https://forms.nh.gov/licenseverification/Search.aspx

Step 2: Click on “Search for a Business License”, written in blue, immediately above “*=required field” and the Criteria Search box.

Step 3: You are now on the “Business Search” page: https://forms.nh.gov/licenseverification/Search.aspx?facility=.

For a Nursing Home

From the “Business Search” page: https://forms.nh.gov/licenseverification/Search.aspx?facility= (see Step 1 through Step 3 above if necessary).

Step 4: Choose “Health Facilities” as the Profession from the drop-down list.

Step 5: Chose “Nursing Home” as the License Type from the drop-down list.

Step 6: Click the blue “Search” tab.

Step 7: Click on the link for the facility to see the licensing information.

For an Adult Residential Facility

From the “Business Search” page: https://forms.nh.gov/licenseverification/Search.aspx?facility= (see Step 1 through Step 3 above if necessary).

Step 4: Choose “Health Facilities” as the Profession from the drop-down list.

Step 5: Chose “Community Residence (4+ beds)” as the License Type from the drop-down list.

Step 6: Click the blue “Search” tab.

Step 7: Click on the link for the facility to see the licensing information.

For New Hampshire Hospital & Hampstead Hospital

From the “Business Search” page: https://forms.nh.gov/licenseverification/Search.aspx?facility= (see Step 1 through Step 3 above if necessary).

Step 4: Choose “Health Facilities” as the Profession from the drop-down list.

Step 5: Chose “Hospital” as the License Type from the drop-down list.

Step 6: Click the blue “Search” tab.

Step 7: Click on the link for the facility to see the licensing information.

For a Youth Residential Facility

From the “Business Search” page: https://forms.nh.gov/licenseverification/Search.aspx?facility= (see Step 1 through Step 3 above if necessary).

Step 4: Choose “Child Care” as the Profession from the drop-down list.

Step 5: Chose “Residential Based Program License” as the License Type from the drop-down list.

Step 6: Click the blue “Search” tab.

Step 7: Click on the link for the facility to see the licensing information.
