A Long Fought For Homecoming

We began working with Toni several years ago, after receiving a call that she’d been hospitalized at New Hampshire Hospital for over 20 years.  Toni wanted to return to her community but wasn’t able to access the supports she needed so she remained institutionalized, year after year.

Our attorney met with Toni and began advocating on her behalf. Toni’s case was complicated and there were several legal and practical hurdles to overcome. What was clear, was that there was no medical or clinical need for Toni to remain at New Hampshire Hospital. With the appropriate community-based services in place, Toni could successfully transition to a community-based setting. Finally, after years of advocacy by us, her guardian, and several other agencies, Toni was discharged to a small residential community home.

She’s loving her new life outside of the hospital walls and at home in her community.
